Greetings! Thank you for making an appointment for a nutrition consultation.
Please bring the following with you to your first appointment:
- Your insurance company referral and co-pay OR the fee for an initial visit: $120.00 cash, check, or credit card due at time of service to receive "Prompt Pay discounted rate.
- A list of everything you ate and drank for 3 days. Try to list amounts and time of day.
- A list of the exercise you did in the past week.
- A copy of your latest lab work if you are seeing me for high blood pressure, cholesterol or Diabetes. Your physician is welcome to fax your lab work to me at 703-644-4461 at least 24 hours in advance.
24 Hour Cancellation Policy
Since our meeting is scheduled for 50-55 minutes,
I need advanced notice for any cancellation.
Please call me 24 hours in advance or more.
There is a $50. charge.
